We have upgraded ordering to accept payments via PayPal. You may use Visa / Mastercard or PayPal for payment. You may still use the order form for ordering more than one item or custom orders.

How to place an order with the order form:
1) Download the Order Form (PDF)
2) Print the Order Form
3) Fill in the Order Form with desired merchandise and quanity
4) Total your order with tax, shipping and handling charges
5) Send complete Order Form to the address on the form
6) Enclose Cash, Check, or Money Order payable to Zombieye
7) Wait for your Voodoo Doll and/or merchandise to arrive
